Big Toe Crush Injury

History: Adolescent girl dropped a weight on her foot with bone visible on physical exam. 

Right toe crush injury: radiograph of the right foot shows an obvious defect at the distal first digit of the right foot (yellow arrow), with underlying destruction of the tuft of the first distal phalanx.

Right toe crush injury: radiograph of the right foot shows an obvious soft tissue defect at the distal first digit of the right foot (yellow arrow), with underlying destruction of the tuft of the first distal phalanx.

This is the appearance of a crush injury to the first toe. This patient dropped a 25 lb dumbbell weight onto her first toe!
